GOSHEN, N.Y. (February 14, 2012) – Residents of four Elant communities in Dutchess and Orange counties will soon reap the benefits of enhanced personalized care and around-the-clock coordination of medical care and other services through ArchCare Advantage, a Medicare Advantage HMO Special Needs Plan.
Elant, the Hudson Valley’s premier provider of senior health care and housing solutions, has entered into an agreement with ArchCare to offer ArchCare Advantage to residents of Elant at Fishkill, a 160-bed skilled nursing care and rehabilitation center located in Beacon. A representative from ArchCare has been on-site for the past several weeks and has started enrolling Elant at Fishkill residents into the program. ArchCare Advantage will be phased into other Elant communities in Wappingers Falls, Goshen and Newburgh as agreements are signed later this year.
Specially designed for nursing home residents, ArchCare Advantage combines solid, ongoing preventive care with individualized care for those with chronic health conditions. Members receive personalized attention from a nurse practitioner/care manager who monitors their health on a daily basis and coordinates all aspects of their care with the nursing home. The program aims to prevent avoidable hospitalization visits and the stress associated with them by intercepting and addressing potential health issues before they become more acute problems.
In addition to the benefits provided by traditional Medicare, ArchCare Advantage provides coverage for prescription drugs and a range of other features. These added benefits include no-cost primary care and specialist visits during 2012, unlimited skilled nursing days without a required hospital stay and a generous additional bed-reserve benefit that guarantees a nursing home bed will remain available should the member need to leave the nursing home temporarily to receive care in another institution for any other reason.
ArchCare Advantage does not require any additional monthly plan premiums beyond the cost members already pay for Medicare, including any Medicare Part D drug coverage.

The ArchCare Advantage HMO Special Needs Plan for Nursing Home Residents currently has more than 1,000 members residing in eight nursing homes in New York City, Westchester and the Hudson Valley.
