Managed Long Term Care Program grows to meet demand of local communities
GOSHEN, N.Y. (June 5, 2012) – Elant, the Hudson Valley’s premier provider of senior health and housing solutions, is pleased to announce that its Managed Long Term Care Plan, known as Elant Choice, has received New York State Department of Health approval to expand into Dutchess County, effective June 1.
Elant Choice is a Medicaid Managed Long Term Care Plan (MLTCP) that provides long term care services to eligible adults in their homes. Elant Choice currently operates in Orange and Rockland counties. The expansion of Elant Choice into Dutchess County will address a growing need for managed long term care in these communities. The program provides an array of covered long-term care services to its clients through their provider network which includes, but is not limited to; case management, adult day care, dental care, medical equipment, home delivered meals, and home health care including skilled nursing, home health aides, and occupational, physical and speech therapy. The goal of Elant Choice is to help clients remain safely in their homes for as long as possible; delaying or preventing placement in a nursing home.

Each client enrolled in the Elant Choice plan, receives a personalized care management plan developed in conjunction with the client and the Elant Choice case manager to address the client’s individual needs. Clients receive services in their homes or through one of Elant’s contracted day care centers.
“The mission of Elant Choice is for our clients, many who are managing chronic disease, to remain independent and active, while living in their own home,” said Deborah DeJesus, Administrator of Elant Choice. “Through the services provided by our program, we allow them to do just that, while ensuring they receive the health care they need on a daily basis. We’re thrilled that individuals in Dutchess County will now have access to our high-quality services.”
You are eligible for the Elant Choice Program if you are:
• At least 18 years old;
• A resident of Orange, Rockland or Dutchess County;
• Eligible for Medicaid or willing to pay privately (available in Orange and Rockland County only) if Elant Choice has not reached its private pay participant enrollment limit;
• Eligible for nursing home level of care;
• Capable of returning to, or remaining
in your home without jeopardizing your health and safety; and
• In need of health and long-term care services from Elant Choice for more than 120 days.
